It started by asking my age and gender. I was asked several questions relating to my diet, and then questions regarding my activity level. I finished off the questionnaire by answering questions about my lifestyle. When I completed the assessment a customized plan was created for me advising me which supplements would be most beneficial for me, foods to add to my diet, and suggestions to include more exercise into my lifestyle.
